 Physician's Medical Clearance Form

This form only needs to be completed by your physician if a current sports physcial form is not on file with our school nurse. This form needs to be returned to Mr. Michael Murray, Director of Physical Education, Health, and Athletics. Any student that has a valid sports physical does not have to return this form.
Participation Form
